Anyway. I usually look for anything that's
fun,
interactive, and
creative. Unique, pleasant systems are nice, and graphics are great, especially if they look good.
1. Fun - If you do not know what fun is, I will cry. The fish quest on 2k1 was fun.
Being forced to work together with other players is fun, especially if they're your friends. Guild wars are fun. Roleplaying with people who can actually roleplay is fun.
Pressing D 5326 times to make 10 gralat is not fun.
2. Interactive - You know, I haven't really seen any playerworlds with a lot of interactive NPCs. Sure, it takes a lot of time and effort, but how about making an NPC say different things according to your statistics? How about being able to actually talk to the NPCs?
3. Graphics - Take a look at Oasis and Murasamune. If those aren't great graphics for a playerworld, I don't know what would be. I love the great, 'bouncy' looking graphics of Oasis, and Murasamune's graphics would be great for some serious roleplaying or questing.
4. Creativity - Probably the main reason whether or not a playerworld goes up. I mean, who really wants 235 clones of Classic? You've already got UN, N-Pulse, Delteria, and all of that other stuff... all of them are basically the same thing. That's why I'm really looking forward to Oasis, Krynn, and all of those other creamy-filled playerworlds.

I mean, suuure... nobody likes a quest filled with baddy-slaying, but why not add a few lizardons here and there to spice up the storyline? Why not make a mini-game or mini-quest as to which you have to help a town blow up a small mountain? I'm pretty sure that would be fun. (maybe not to script, but hey...)
A little hint: Making a playerworld with 'unique' sparring rooms, levels, and everything like that will most likely not make your playerworld pass. Especially through me.
...the bad part about what I said is, most of those things are hard to script...
