Low you need 100/50 (minimum)
Medium you need 300/150 (minimum)
and High you need 600/300 (minimum)
minimum meaning atleast 50 grace, atleast 150 grace, and atleast 300 grace to get those relics/prayers.

So...how do we gain these spells? "Without praying on an altar," do they just add themself to our spell list after we reach that level?

You worship a god.
Checkout the Gods forum to see which prayers which gods grant.
Once you get the specified grace, you pray, and pray, and pray, until you get the prayer.
