Sounds like a good layout.
But maybe make it worth more... Overall should count on all the things together so....
Make the Gameplay,Concept,Innovation,Design and Visualization out of 10 each (1 being worst 10 being best).
Tally all the "points" at the end out of 50 then multiply it by 2 to get a mark. If the pw is under a cetain amount like 70 or 80 (cause we want good pw's) then they get a fail if its above 70-80 then they pass.

Ex:

Gameplay - 5
Concept - 3
Innovation- 2
Design - 7
Visualization- 5
Overall: 22 x 2

Status: Fail

Then for each part say why you graded them like that and how they can improve etc..
