Having regularly gotten so deep in this dungeon that only 2 monsters spawn at a time, I can say that this does happen... very rarely.

Maybe one every half dozen levels a monster will spawn on a wall or something.. but generally you're using a bow for most effective clearing anyway. Rarely, maybe 10% of these will spawn so far away that you can't get them... in which case you can just spam bullet storms and get them. In the superduper rare case this monster is a vampire guard... well just restart cause that's like 1/10000 levels.

The cracks spawning on walls was fixed pretty effectively by Deas last I checked, I used to be prohibited from going more than 30-40 levels down, right when you start getting rewarded with max monster spawns of like 10~. Now I constantly always get to 2 monsters, every time, and can go for hours until I get bored. Unless something changed..? the cracks should always be within skii range.
