ive never seen a 2d horror game at all realistic or not in graal perspective
iam not even sure if you can do any fear effects with it
i saw some rpgmaker games with blood dead bodys and other things but these
was more funny than mature

so i think the best is we wait for someone who reaches the point with some example
even if they allow things yet they can always change the rules
