Use this form to complain about Events Team members:

1.) What is your account name/nick name/E-mail address?
2.) Who is the ET in question? (Acct:Nick)
3.) State your complaint (Be logical, please.)
4.) List atleast 5 witnesses. (Account:Nickname:E-mail address)
5.) If the person is a repeat offender, list the number of times you have made a complaint of the same questionable event.
