Do you simply press 'S' and your sword activate or like Graal2002 sword command where you only allow to use your sword when you draw it out?

You are allowed to equip weapons, such as a sword. When you equip a sword, it will be in your scabbard. So yes, you will pull it from the scabbard before being able to use it. (also scabbard must be obtained before using a sword)
